Weight Loss Clinic – Terms and Conditions

 

1.    Ongoing Treatment and Review Schedule
All patients undergoing weight loss treatment at our clinic will be reviewed at a minimum every 12 weeks (3 months). These reviews are mandatory to ensure the safety and suitability of ongoing prescribing and to assess progress toward treatment goals.


As part of this review process, the prescriber may request a face-to-face consultation if clinically indicated. This will be determined on a case-by-case basis and is at the discretion of the prescriber.

 

2.    Monitoring Progress and Milestones
As patients approach their goal weight or reach a BMI of 30 or below, further evidence of progress may be required. This may include, but is not limited to:

    •    A photograph of current weight on a set of calibrated scales
    •    A full-length photograph in tight or form-fitting clothing to assess physical changes
    •    An updated consultation form disclosing any changes in health or medical history
 

3.    Prescriber’s Discretion
All decisions regarding the continuation of treatment, pausing of medication, or transitioning to maintenance dosing are made solely at the discretion of the prescriber, Tristan [Surname].
Even if a patient has reached a BMI of 30 or below, the decision to continue treatment on a maintenance basis is not automatic and remains at the prescriber’s clinical discretion, based on individual circumstances and overall health assessment.

 

4.    Medical Honesty and Responsibility
It is the patient’s responsibility to disclose any new or existing medical conditions during the consultation process. Failure to provide accurate or complete information may result in treatment being paused or discontinued in the interest of patient safety.

 

5.    Right to Refuse Treatment
The clinic reserves the right to withhold or discontinue treatment where it is deemed no longer clinically appropriate, or if required information is not provided within a reasonable timeframe.

 

6.    Payments and Refunds
Any deposits or payments made in advance of a prescription request will be fully refunded if treatment is refused, paused, or deemed clinically inappropriate following a review.

Refunds will be processed within a reasonable timeframe and returned via the original payment method.
